Please fill out the form below for the consideration for financial assistance through Reach PV Church. The financial assistance will allow Hope Gatekeepers' certified Crisis Specialist and Suicide Prevention Instructors to provide its service(s) to you or your loved one.

An average of two L.A. County residents dies by suicide every day. This rate is higher than the rates of drug overdose, car accident fatalities, and murder.
